Booths are 6 feet by 6 feet and cost $375. It includes a 6-foot table and chairs. Cost includes booth space and CPIA membership for 2008. Please indicate if you need electricity. ___YES ___NO
Exhibitors may put an 8 ½ x 11 inch black & white ad in the Proceedings at no charge. If you are unable to be an exhibitor but would like to put an ad in the Proceedings, it will cost $80. Ads must be received by Jan. 30, 2008. ______Yes, I want an ad. NEW THIS YEAR A Power Point presentation that includes 3-5 images of exhibitor products will be displayed this year. If interested in this activity, contact Donna Lamm prior to Jan. 30.
CPIA will be giving away door prizes throughout the day. If you would be willing to donate something for a door prize please check here. ______
The person listed above receives free registration with the purchase of a booth. If you are bringing additional staff, please list them below. Registration is $65 for each additional person.

A block of rooms has been reserved at the Regency Hotel, Greeley, CO. The cost is is $69.99/night for single occupancy; $71.99 for double occupancy. Both smoking and non-smoking rooms are available. Please make your reservations directly with the motel at 970-353-8444. Indicate you are with CPIA to get the special rate. The block will be released Jan. 18, 2008.
